What is an Assisted Living Waiver Program?
The Assisted Living Waiver (ALW) is a legislative initiative designed to offer assisted living care services as a Medi-Cal benefit. For additional information, please visit the Department of Health Care Services.
What are the eligibility requirements?
- Be 21 years or older;
- Maintain full-scope Medi-Cal eligibility with no share of cost;
- Enroll in SSI/SSP/SSD or receive a monthly income.
- Demonstrate care needs equivalent to Medi-Cal-funded residents in Nursing Facilities;
- Express a preference for residing in an assisted living environment rather than a Nursing Facility;
- Have the capability to live safely in an assisted living facility or publicly subsidized housing;
- Express a willingness to live in an assisted living setting within specific counties offering ALW services, including but not limited to Alameda,Contra Costa, Fresno, Kern, Los Angeles, Orange, Riverside, Sacramento, San Bernardino, San Diego, San Francisco, San Joaquin, San Mateo, Santa Clara, and Sonoma.
Can I apply without Medi-Cal benefits?
Certainly. If you furnish your social security number and date of birth to us, your Care Coordination Agency (CCA), we can include you in the waitlist. Enrolling in Medi-Cal is necessary by the time we conduct the assessment.
How does the application go?
Complete and submit the application packet to determine if you meet the pre-eligibility criteria. If all pre-eligibility criteria are met, CENTRAL COAST HEALTHCARE SERVICES INC will assist you with the enrollment process for ALW at no cost.
